How to add 'File Input' to a form

Can somebody tell me how to input a file (jpg gif png pdf) in a form, and summit it to FormMail or PhpFormMail to be redirected to the recipent indicated in the form by the person who filed in the form? The idea is to allow visitors of my page to be able to send messages with attached files without using public web caf? programs or at their's friends place.

Don's version of PHPFormmail will allow you to attach files.
